  6. Scheme certificate if any, employer having (Previous employer pension certificate).

  8. Original cancel cheque of State bank of India / Punjab National Bank salary account.

  

  • If an employee is retired from his job and claiming pension after 58 years of his/her age then the employee would require to submit the below-mentioned documents: EPS Pension Form 10D
  1. Form 10 D 3 Set original signed by the employee.
  2. Pan card, Aadhar card, Other Id proof (Name, Father Name, Date of birth should be same in all the ID proofs)
  3. Employee’s Birth Certificate / 10th Mark sheet copy for Age proof other documents are not acceptable.
  4. Nominee Id proof (Aadhar card, Pan card, Voter ID)
  5. Below 25year Children’s Birth certificate / 10th Mark sheet copy / School certificate copy for Age proof.
  6. No pension certificate (Declaration pension not getting)
  7. Scheme certificate if any, employer having (Previous employer pension certificate).
  8. Joint family photograph color 3 copy passport size.
  9. Employee mobile no. Height and Identification marks.
  10. Original cheque of State bank of India / Punjab National Bank.

If an employee is married then claim should be sent by the wife and children of the deceased employee

  1. Form-20, 1 set, and Form 10D 3 set original signed by Employee’s wife (Nominee), not children.
  2. Employee Id proof(Pan card, Aadhar card, Driving license, other ID proof.)
  3. Original Death certificate. (Post mortem report and FIR copy.)
  4. Wife(Nominee) ID proof (Pan card, Aadhar card, Driving license, other ID proof.)
  5. Below 25year Children Birth certificate / 10th Mark sheet copy / School certificate copy for Age proof.
  6. Affidavit copy (Pension can only be claimed by the Wife/Children of the claimant in case of death. Wife/Children needs to declare their dependency on employee stating that they are not getting any kind of monetary benefit from any other department and also need to share the work profile of employee’s last organization )
  7. Joint family colored photograph 3 copy passport size.
  8. Nominee’s mobile no. Height and Identification marks.
  9. Original cancel cheque of State bank of India / Punjab National Bank. All family members’ bank account should be in the same branch and IFSC code will be the same.

 

  • If an employee is non-married then the claim should be sent by the mother and father of the deceased employee. EPS Pension Form 10D
  1. Form-20, 1 set and Form 10D 3 set original singed by Employee’s Father (Nominee) not Mother.
  2. Employee Id (Pan card, Aadhar card, Driving license, other ID proof.)
  3. Original Death certificate. (Post mortem report and FIR copy.)
  4. Father and mother (Nominee) ID proof (Pan card, Aadhar card, Driving license, other ID proof.)
  5. Age proof birth certificate / 10th Mark sheet copy / Declared by Village Sarpanch / MP/MLA On letterhead.
  6. Affidavit copy (Pension can only be claimed by the Father and Mother of the claimant in case of death. Father and mother need to declare their dependency on employee stating they are not getting any kind of monetary benefit from any other department and before joining at organization employee was what to do.)
  7. Joint family photograph color 3 copy passport size.
  8. Nominee mobile no. Height and Identification marks.
  9. Original cancel cheque of State bank of India / Punjab National Bank. All family members’ bank account should be the same branch and IFSC code will be the same.

  1. I have retired since 01 March 2020 having reached the age of 60 years. My employer has submitted the FORM 10D to the EPFO, however I have not received my Pension Payment Order Number details.
    How can I obtain my PPO number details.
    Tried getting PPO details through GOI Digi Locker and also through EPF India – Pensioner’s Portal through Bank Acc. Search and Mem. ID Search both were unsuccessful.
